CRB Checks

An evening has been arranged at Trent Bridge for you to get your CRB signed off before the start of the season.

Date: 22nd March 2011


Time: from 6pm to 7.30pm

Venue: New Board Room, 2nd Floor, New Office Block, Trent Bridge Cricket Ground.

Please email chloe.gibson@nottsccc.co.uk with a time you will be coming. 

Below is a list of the positions that require a CRB check, any others will not be recognised.

Welfare Officer
Colts Manager
Coach
Age Group Manager
Assistant Coach
Club Captains
Coaching Co-Ordinator
Adult Team Captains
Umpire
First Aiders/Physiotherapist/ Medical Support
Scorer
Schools Liaison Officer
Women & Girls Co-Ordinator


YOU MUST bring a NEW (purple) CRB form with you. Unfortunately we will not have any on the night. To obtain a CRB form please contact TMG on 0115 969 4606 and ask to speak to the ECB department

You will need to ensure the following is completed on the form:

- Completed using BLACK INK

- Complete all sections highlighted in yellow. Please write neatly, keeping letters inside each box

- Put a line through any mistakes and write the correction to the right. Do not use correction fluid.

- Ensure National insurance number is on the form

Bring with you: 3 of the following documents

·     Driving licence

·     Birth Certificate

·     Passport

Below must be within the last 3 months

·     Utility bill (electricity, gas, water, telephone, mobile phone contract/ bill

·     Credit card statement / Store card statement

·     Addressed payslip

·     Bank or Building Society statement
